Proctor Site Request Form

Students living more than 100km away from the University require a proctor. You must submit this form within 2 weeks of the start of term.

Proctoring sites are located in accredited colleges or universities. Under no circumstance may a proctor be a close friend, relative, or family member of the student. Unless there are circumstances beyond your control you must use the same proctor site throughout the academic term.

NOTE: Some midterms & final exams may be online. Please ensure your proctor site has internet access.
